In order to make valid application for admission to Madison Avenue Baptist Academy the following items must be completed and submitted:
One (1) student/guardian questionnaire for each student seeking admission.
One (1) student application for each student seeking admission.
Copy of studentís full immunization record from physician or nurse of studentís previous school.
Copy of the studentís report cards or permanent records, a minimum of two previous years, along with standardized test results.
Disciplinary Records (If Applicable)
Copy of any disciplinary records the student may have on file at previous school.
Upon receipt of the information detailed above and completed scholastic testing you will be notified of admittance by the school office.
